Johns Hopkins will grant exceptions to the COVID and/or influenza vaccination mandate for medical reasons, with appropriate documentation.
Requests for an exception to the vaccine mandate must be made through the Johns Hopkins Vaccine Management System (VMS).
Log into VMS and select Request exception.
Select any/all vaccines for which you are seeking exceptions from the text box options. Select the Medical radio button.
A pop-up screen will appear indicating that you must download a medical exception form for whichever vaccines you selected, and ask your healthcare provider for a supporting letter. Once you download the form, press Next.
Once your forms and supporting documents are completed, enter your contact details and your health care provider’s details. Choose documentation files to upload. Note the documentation guidelines. Check the Verification and Accuracy box and press Submit.
Your documentation will be reviewed by a qualified health services staff member. Once validated, you will receive a separate confirmation email with further details. If you have an approved medical exemption to the COVID vaccine, it carries over to the booster requirement. You do not need to apply again.
